Use our referral form OR submit the referral information including patient name, contact information and insurance information; relevant medical history and imaging, current medications; and reason for referral and clinical notes.

Step 2: Scheduling
Once the referral is received, Specialty Pain Care will contact the patient directly to schedule an appointment.

Step 3: Provider Communication
Referring providers will receive updates regarding findings, treatment plans and follow-up recommendations.
